On 05 September 2017, the bulk carrier "BOTAFOGO SW" reported that its life boat with 2 people on board, dropped from a meter above the waterline into the sea due to failure of its release hook, while carrying out safety drills at English Bay anchorage 2, Vancouver, BC. The two crew members who were trapped in the capsized life boat were recovered from the water by assist vessels. Damage and injuries reported.
